Program Description: We offer free health care to adults 18 and up: Free healthcare, Work physicals, Vision and Dental referral, Diabetes screening, Social Workers, Counselors who work with the Ohio Benefit Bank, Tax assistance, Legal Aid, Diabetic Foot Checks, Flu Shots, TB Testing, On Site Lab Draws, Medical History and Scribing Training Classes on our EMR System, Chiropractic Care, Spiritual Guidance, Hypertension Screening, Health Care Navigation (Affordable Care, Medicare, Medicaid), and Free Hugs!
Eligibility Requirements: Adults who meet 200% of the Federal Poverty Guidelines and under.
Hours: Clinic is from 2:30 until last patient is seen. (Please see scheduled clinic dates on our website) Physicals are on Fridays, only on clinic dates, TB Test are on Wednesdays twice a month from 10:00 A.M. – 2:00 P.M. (Please see scheduled TB dates on our website)
Intake Process: Patients are not scheduled. First come first served basis. Wait in line prior to 2:30 opening
Required Documents: Some form of identification with your name on it
Fees: The clinic is free to all who qualify. TB Test is $8.00 Cash. Flu Shots are free when available
Additional Languages: Please bring an interpreter. We usually have Spanish interpreter available.
Area Served: Any adult who is in need of our services
