Program/Service Name: Franklin County Office on Aging – Senior Options
Description: Franklin County Senior Options, a levy funded system of care for older adults and their families, provides information and advocacy, case management, homemaker, medical transportation, adult day care, home delivered meals, minor home repair, personal care, emergency response systems and respite care.
Eligibility Requirements: Those residing in Franklin County age 60 or over with a demonstrated need are eligible for the Senior Options Program. The amount of subsidy available depends upon a financial assessment. Services are provided on a cost-sharing basis to those with a demonstrated need using a sliding fee scale. Case Managers look at monthly income and liquid assets and determine, using the sliding scale, if the participant will be responsible for a portion of the cost of service.
Hours: Case Managers accept calls M-F; 9:00 am – 4:30 pm; Thursday 9:00 am – 7:00 pm

Required Documents: Documents supporting monthly income and liquid assets
